GENERAL INFORMATION

Applying for any of our programs is simple. As you complete the application, please follow the instructions and submit or mail the application with required fees before the admissions deadline.

Please find below the minimum requirement for each program before beginning your application process.

MINIMUM REQUIREMENTS

Practical Nursing Program:  High School/GED or higher education 
Nursing Assistant Program: There are no minimum requirement to enroll in this program, however, we strongly recommend that students are proficient in the English language.
Phlebotomy Program: High School/GED or provide evidence of some high school education or obtain permission from the Director of Education.
Medication Aide Program: Certified Nursing Assistant or must have completed 40 hours DIRECT CARE OR STAFF CARE TRAINING
 

ADMISSION PROCESS FOR PRACTICAL NURSING PROGRAM

Walk in, call or visit our website to obtain information about the program.

  1. Complete the application form online or download form from our website at www.ultimatehealthschool.com. Attach the required registration fees if you complete the form by hand and mail your application to the address below:

NOTE: If you apply online then you have to either pay the registration fee by phone or make provision to do so the day you come and take your entrance examination.

ENTRANCE EXAMINATION

All interested applicants must take an entrance examination and achieve a GED level one in both the quantitative and verbal portion of the exams. Students can take the entrance examination only twice. Please call our front desk(Tel: 703-933-9430 ext 0) and request for sample questions before taking the examination.

Entrance examination fees----------------------------------$30.00(per exams)

ADDITIONAL DOCUMENTS TO BE SUBMITTED:

  • Valid I.D. (e.g. passport, driver’s license, state ID) 
  • Social security card
  • High School Transcript or GED Transcript or Higher
  • Valid CPR (American Heart Association or Red Cross, school provides service at a fee).
  • Mandatory Criminal Background Check
  • Mandatory Drug Test
  • Two (2) letters of recommendation (please find below or click attached the instruction for the recommendation letters)
  • Sign an enrollment agreement form (includes payment options and other cost student will incur)
  • Submit the Health Requirements
  • TB (PPD test/ chest X-ray) - No more than 1 year
  • Hepatitis B (Must be waived in writing) 
  • MMR (MEASLES, MUMPS RUBELLA) 
  • Varicella (CHICKEN POX)

IMPORTANT INFORMATION ABOUT TRANSCRIPTS

Ultimate Health School will only accept original transcripts from applicants. Unofficial transcripts may be accepted while waiting for the original transcript.

INTERNATIONAL TRANSCRIPTS

All international transcripts must be evaluated by one of the following agencies below.Each agency listed charges a fee for its services; the fees vary. The cost for this service is the student's responsibility. Suggested agencies for international transcript evaluation are:

TRANSFER STUDENT

Applicants with an academic letter grade of B or better can transfer only the following courses:

Introduction to Nursing, English Proficiency I and II, Anatomy and Physiology, Nutrition & Diet Therapy, and Pharmacology & Medication Administration

All other courses are not transferable.

NOTE: Applicants who submit transferable courses with a letter grade of C may ask permission from the Director of Education to take a challenge exams. Based on the applicant's performance, the challenged course may be transferred..

INSTRUCTIONS FOR THE TWO RECOMMENDATION LETTERS

Ultimate Health School requires two (2) letters of recommendation from all students admitted into our practical nursing program. A reference can come from an employer, a clergy/Imam/Rabbi etc, a colleague, or any reliable person [preferably an adult friend] that knows applicant at least 2 years or more.

NOTE: FAMILY MEMBERS AND RELATIVES EXAMPLE (FATHER, MOTHER, AUNT, UNCLE, SIBLINGS, AND GUARDIAN, ETC.) CANNOT PROVIDE REFERENCE LETTERS.

The person providing the reference must include in their letter:

  • How long they have known the student 
  • Their relationship with the student
  • Why they believe the student can be a nurse
  • Please address this letter to the Director of Education[Arangu Lydia Ngundam, RN, MBA


ADMISSION PROCESS FOR ALL SHORT PROGRAMS

Applicants interested in our Nursing Assistant program, Medication Aide Program, Personal care Assistant program and Phlebotomy program must:

  1. Visit our website, walk in or give us a call to obtain information about our programs
  2. Request application form from the school or download form from our website at www.ultimatehealthschool.com.  Fill the form and submit it with the appropriate registration fee
  3. In addition applicants must furnish the school with the following documents:
    Valid I.D. (e.g. driver’s license, state ID, Valid Passport)
    Social Security card

    All applicants interested in mailing their applications must send it with the correct associated fees to:
